You'll first create a user name and password. The system will allow you to log out and come back later to complete the application. Once you complete the application, choose and complete the appropriate Skills Checklist.
Before you can be placed in an assignment, you will need to provide the following documentation:
- Statement of Good Health (completed within the last year)
- TB test results (completed within the last year)
- Chest X-Ray results (for positive PPD, within the last 3 years)
- MMR vaccine record or Measles, Mumps and Rubella titers
- Hepatitis B vaccine record or Hepatitis B titers
- Varicella (Chicken Pox) vaccine record, history of the disease or titers
- Professional license and/or certification
- CPR card (American Heart Association required)
- Other certifications (ACLS, PALS, etc.)
- 2 pieces of government or state ID (driver's license, social security card, passport, etc.)
- Picture for your identification badge
